Complimentary Phone Consultation
Before scheduling an in-person appointment, I ask you to complete a free 20-minute initial phone consultation. During this call I will ask you questions about your main concerns for your child, I will provide information about my services and space, and together we can determine if I am a good fit for your needs.
Paperwork
If my services seem well suited to the treatment you are seeking, I will send you an electronic copy of our intake paperwork to be completed before you attend your first appointment. I use a secure portal to send and receive documentation so that we may keep your information private and confidential.
Fees:
0-6 yr. old Assessment & report: $950
EASI/SI Assessment and report: $1000
Modified Assessment (summary report only): $550
Feeding Assessment & report: $475
Feeding Assessment and verbal summary: $375
OT treatment $180 for 50 minutes / $108 for 30 minutes
Feeding treatment $185 for 50 minutes / $111 for 30 minutes
Consultation (one-time appointment): $200/session (60 minutes)
Reports and home programs $180/hour (billed in 15 minute increments)
Travel time: $180/hour (billed in 15 minute increments)
Waitlist
I try to keep waitlists short. If I do not currently have an appointment time that is convenient for you and your family, I will provide you with the contact information of alternative providers in the area.
